Statement of leave

• Advise trainee to upload the statement of leave in the 'Course/ Certificate' area of Learning Log on ePortfolio.  This form can be downloaded from the trainee ePortfolio section on this site.

Evidence checklist and feedback sheet

• Give your trainee a copy of this document so they are aware of what will be checked on their ePortfolio at the ARCP panel.  This document includes a list of WBPA required for each ST year.
